Chandler Hall provides different types of Home Health Care Services according to the individual need. Rehabilitative or Restorative Care is available to people recovering in the comfort of their own homes. A team of registered nurses, social workers, physical therapists, occupational therapists, speech therapists, and home health aides provides visits as necessary to assist participants in regaining their health and returning to their fullest potential.
Special emphasis is placed on educating the participant & their families to promote independence. A dedicated staff works with family members to assess a person's history, strengths, accomplishments and goals to develop an individual plan. Individual programs include primary health care, education, counseling, & therapies. We value the family's role in caring for an individual who is receiving care or coping with an acute or chronic illness.
Home Health Care Programs provide:Our Medicare Certified Home Health Program is JCAHO certified. Following a medical occurrence, with a Physician's order, we are able to provide clients with therapies and nursing visits. Home Health visits are processed through your insurance plan. Chandler Hall accepts most health insurance plans and is a provider of Home Health in Bucks County.
